> Edad Requisito 2.9 - 4.7 ( Cumplidos Para Agosto )
> Certificado de Nacimiento (Original)
> Certificado de Vacuna (Hoja Verde)
> Verificación de Ingresos de los Padres
> Dos (2) fotos del Niño/a (2x2)
> Seguro Social del Niño/a
> Tarjeta de Seguro Médico y MA-10 o Plan Privado
> Verificación de Residencia (Talonario de agua, luz o teléfono)
> Diagnóstico del Niño (si tiene alguna necesidad)
> Requisitos de salud: Laboratorios: CBC o hematocrito, prueba de plomo, examen físico y dental

> Birth Certificate (Original)

> Certificate of Vaccination (Green paper)

> Verification of parental income

> Two (2) photos of the Child (2x2)

> Child's Social Security 

> Health Insurance Card and MA-10 or private medical plan

> Verification of Residence (water, electricity or phone bills)

> Diagnosis of the Child (if necessary)

> Health requirements: Labs: CBC or hematology, lead testing, physical and dental examination
